Hello
from the Lakeshore Community Healthcare, School Dental Program!  This
dental program was utilized this past Fall in the Sheboygan Falls School
District.

Lakeshore Community Healthcare (LCHC) and the
Sheboygan Falls School District have partnered together to provide a dental
clinic in the elementary, middle, and high schools.  This school
program offers x-rays, cleanings, exams, fillings, sealants and other dental
services as needed, for all students in the district who are NOT already
established with another dentist.

LCHC accepts private insurance and
Medicaid/Badgercare. Help is available for those students not covered by a
dental insurance plan.

If you would like your child to take
advantage of this opportunity, please complete and return our bright green
registration forms available in the school offices or from the school nurses.

We plan to return for our clinic with the
Sheboygan Falls schools starting the week of March 19th.  Please
return completed forms to your child’s teacher or to the school’s main
office by March 1, 2017.  If you have any questions please call
Clare Spaulding, LCHC School Dental Program Coordinator, at
920-783-6633.
